WHEN TO USE IT
Complete this form in NJ SPIRIT. The Visitation Plan is completed for all children in out-of-home placement, even when there is a court order prohibiting or limiting visitation or other circumstances warrant visitation suspension. See CP&P-IV-A-5-100. The Visitation Plan is used to document:
� �� The visitation plan for the first visit between the child and his or her parent(s)/caregiver(s);
� �� The visitation plan for ongoing visits between the child and his or her parent(s)/caregiver(s), siblings, and other relatives;
� �� The renegotiated visitation plan between the child and his or her parent(s)/caregiver(s), siblings and other relatives;
� �� Any change or reduction in the frequency or duration of visits approved by the Supervisor
HOW TO USE IT
Complete the form in the NJS Case Plan window by accessing the form through NJ SPIRIT desktop > Create Case Work > Case Planning/Family Assessment > Case Plan > Visitation Plan Tab > Options Drop-down.
The CP&P Form 26-83 may also be accessed through the on-line Forms Manual, to view or print a blank version of the form.
