WHEN TO USE IT

 

Complete this form in NJ SPIRIT.  CP&P Form 26-87 identifies the strengths and needs of the family and the specific activities necessary to achieve the desired family outcomes.  Document activities and strategies for achieving these outcomes, as well as the estimated completion dates on this form.  A Family Team Meeting participant is designated to monitor and evaluate the progress toward achievement of the desired outcomes, for each family member.  This form is used in conjunction with the CP&P Form 26-81a/b, Family Summary/Case Plan.

  

HOW TO USE IT

 

Complete the form in the NJ SPIRIT application, by accessing it through the NJS Desktop > Create > Case Work > Case Planning/Family Assessment > Case Plan > Case Goals Tab > Options drop-down > CP&P 26-87.

 

TIPS FOR COMPLETING THE FORM

 

Complete the portion of the form that includes the strengths of the family first, then identify the needs of the family.

 

The results of the following approved forms in NJS will prefill the Strengths and Needs Tab of the Case Plan Window:

 

·         CP&P Form 22-24, Caregiver Strengths and Needs

·         CP&P Form 22-25, Child Strengths and Needs

 

In the Strengths and Needs Tab of the Case Plan Window, click the "Edit" hyperlink to document:

-     Desired Outcomes

-     When the Outcome was established: previously, recently, or, deferred to address later

-     Additional Outcome Information

-     Begin Date

-     Estimated Completion Date

-     Information in the Activities Group Box

 

In the Activities Group Box, document:

-     Specific Activities

-     How Progress Will be Monitored, Evaluated, and by Whom

-     Begin Date and End Date

-     Resource

-     Services and Frequency
