WHEN TO USE THIS FORM:

 

This form shall be used upon the initial and subsequent out-of-home placement(s) of a child or adolescent who is age twelve or older, and annually throughout the child or adolescent’s placement.

 

HOW TO USE THIS FORM:

 

The assigned Worker reads this form to the child or adolescent, and ensures the child or adolescent understands his or her rights. The Worker explains any areas the child or adolescent may want clarified. The child or adolescent signs the Acknowledgement of Receipt of Understanding. The Worker makes a copy of the document and presents a copy to the resource family parent and places a copy in the child or adolescent’s case record. The Worker gives the original document to the child or adolescent.
