WHEN TO USE IT

 

Supervisors, Casework Supervisors, County Service Specialists, and Case Practice Specialists who want to serve CP&P as SPRU Workers review this form in full, and sign it, to indicate a thorough understanding of its contents and agreement to comply with its tenets.

 

The agreement must be signed before the Supervisor, Casework Supervisor, County Service Specialist, or Case Practice Specialist can join the local roster and serve as a SPRU Worker for the local operation.

 

When there is a vacancy in the local SPRU roster, the candidate can join the local SPRU team, and sign up to work SPRU Worker shifts on call.

 

SIGNATURES

 

The Supervisor, Casework Supervisor, County Service Specialist, or Case Practice Specialist seeking an opportunity to work as a SPRU Worker signs the form.

 

The form is also signed by the following staff, to confirm their approval of the candidate:

 

·         SPRU Coordinator

 

·         Area Director or designee
