Medical Waste Renewal Questions & Answers

Question:

Why have I not received my Annual Report?

Answer:

GENERATORS WHO DID NOT RECEIVE THEIR MW ANNUAL REPORT OR ARE REQUESTING TRACKING FORMS ARE TO CALL (609) 984-6620, SELECT #1 AFTER LISTENING TO THE RECORDING FOR THE TECHNICAL PROGRAM.

Question:

When will the Medical Waste invoices be mailed?

Answer:

The billing cycle for the medical waste generator registration invoices is July 20, of each year. Invoices must be paid by August 20, of each year. Please allow approximately two (2) WEEKS before calling the Department to report that you have not received your bill.

 

Question:

I received an invoice, but I previously notified your office that I am no longer in practice, what should I do with this bill?

Answer:

Please indicate on the bill that you are no longer in practice and return it in the envelope provided or you may fax (609) 777-0769 it back.

 

Question:

I recently took over another's Doctor's office; can I transfer his identification to my practice?

Answer:

NO. Identification numbers are non-transferable. Call our office for an application or download an application from our website at www.state.nj.us/dep/dshw/. If you need assistance: please give our office a call.

 

Question:

I received my invoice and they have me in the wrong category, what should I do?

Answer:

You can make the corrections on your invoice.

 

Question:

My address changed, how do I notify the Department?

Answer:

All changes must be done in writing. No information is taken over the phone. You can notify us in writing, by mail, or by fax at (609) 777-0769. Attn: Medical Waste. (Please put your generator ID number on all correspondence that you send to our office.)

 

Question:

My Owner/Business has changed but my Federal Tax ID number remains the same, do I have to apply for a new one?

Answer:

NO. But you must notify the Department in writing that your Owner/Business name has changed and specify that your Federal Tax ID number remains the same. For our records, please include your Tax ID number in your letter.

 

Question:

I received 2 invoices but only have 1 location, which one should I pay?

Answer:

Please provide us with both generator numbers so that we can investigate and let you know.

 

Question:

Will we be receiving a certification to post in our office this year?

Answer:

No, Registration certificates are not required to be posted and therefore will no longer being issued.

 

Question:

I paid my invoice by Intra Governmental Transfer. I have received a notice from your Department indicating that my invoice has not been paid. I have proof that this invoice was paid. What's the problem?

Answer:

If you're a Government/State agency who processes and pays yours bills/invoices electronically through an Intra Governmental Transfer voucher system, each year you must send a signed hard copy of that voucher along with your renewal to be scanned for payment. With out this documentation, you will continue to receive outstanding bills/invoices and will be considered not paid.
