“My Personal X-ray Procedure Record”

Thank you for your interest in our attempt to inform the public of the licensure requirements of this State. In an effort to enhance the public’s knowledge about the profession of radiologic technology, help reduce their exposure to ionizing radiation and provide an easy way to track x-ray procedures, we have developed the “My Personal X-ray Procedure Record” card. The front of this card provides the public with seven steps that they can take to protect themselves prior to having an x-ray procedure and information about the licensure requirements for radiologic technologists. The back of this card contains an area for the public to record any x-ray procedures that they may have. We believe that this card can help improve communications between the health care provider and patient, enhance the doctor’s understanding of their patients’ x-ray history and avoid unnecessary duplicate procedures.

PURPOSE:

The Department of Environmental Protection (Department) suggests that all x-ray procedures be recorded as part of a patient’s permanent health records. The Department provides the “My Personal X-ray Procedure Record” card to inform the public of the licensure requirements for all operators of x-ray equipment in New Jersey and to help track an individual’s x-ray procedure history. This card is not intended to suggest a safe limit to the number of x-ray procedures that can be performed. Using this card can help doctors understand their patients’ x-ray procedure history and help to avoid unnecessary procedures.
