National Transition Resources

  • PACER's National Parent Center on Transition and Employment
    PACER Center was created by parents of children and youth with disabilities to help other parents and families facing similar challenges. PACER Center enhances the quality of life and expands opportunities for children, youth, and young adults with all disabilities and their families so each person can reach his or her highest potential. PACER operates on the principles of parents helping parents, supporting families, promoting a safe environment for all children, and working in collaboration with others.

  • The National Center on Secondary Education and Transition (NCSET) - NCSET provides technical assistance and disseminates information focused on four major areas of national significance for youth with disabilities and their families —
    • Providing students with disabilities with improved access and success in the secondary education curriculum.
    • Ensuring that students achieve positive postschool results in accessing postsecondary education, meaningful employment, independent living and participation in all aspects of community life.
    • Supporting student and family participation in educational and postschool decision making and planning.
    • Improving collaboration and system linkages at all levels through the development of broad-based partnerships and networks at the national, state, and local levels.
  • The National Post-School Outcomes Center - Information about the National Post-School Outcomes Center, whose mission is to help state education agencies establish practical and rigorous data collection systems that will measure and profile the post-school experiences of youth with disabilities. The results will be used for national, state, and local reporting and-most importantly-to guide and improve transition services to this population.
  • The National Collaborative on Workforce and Disability for Youth (NCWD/Youth) - Information about the NCWD/Youth, which works to ensure that transition age youth are provided full access to high quality services in integrated settings to gain education, employment and independent living. Goals of NCWD/Youth include; 1) Improving systems capacity to provide comprehensive transition services for all youth, including youth with disabilities and youth from vulnerable populations; 2) Strengthening the services provided by organizations responsible for delivery of workforce development services; and, 3) Promoting the need for and further grow a professional development system for youth service professionals.
  • The National Dissemination Center for Children with Disabilities (NICHCY) - This section of NICHCY's website is devoted to transition issues, resources, and best practices.  It will help you learn more about transition ---what the law requires, what information a typical transition plan contains, how transition plans are developed, and other topics.
  • The National Longitudinal Transition Study-2 (NLTS2) - NLTS2 is a follow-up of the original National Longitudinal Transition Study.  The original NLTS was designed and conducted from 1985 through 1993.  NLTS2 includes 12,000 youth nationwide who were ages 13 through 16 at the start of the study (2000).  Information will be collected over 10 years from parents, youth, and schools and will provide a national picture of the experiences and achievements of young people as they transition into early adulthood.
  • The National Secondary Transition Technical Assistance Center (NSTTAC) - NSTTAC is a national Technical Assistance and Dissemination center directed and staffed by the Special Education Program at the University of North Carolina at Charlotte, in partnership with the Special Education Programs at Western Michigan University and Appalachian State University.  In order to ensure full implementation of IDEA and help youth with disabilities and their families achieve desired post-school outcomes, NSTTAC helps states to build capacity to support and improve transition planning, services, and outcomes for youth with disabilities and disseminate information and provide technical assistance on scientifically-based research practices with an emphasis on building and sustaining state-level infrastructures of support and district-level demonstrations of effective transition methods for youth with disabilities. This web site features research based transition practices, lesson plans, and other materials for parents, students, teachers, and administrators.
  • The Job Accommodation Network (JAN) - JAN's mission is to facilitate the employment and retention of workers with disabilities by providing employers, employment providers, people with disabilities, their family members and other interested parties with information on job accommodations, entrepreneurship, and related subjects.  JAN represents the most comprehensive resource for job accommodations available.
  • The Partnership on Employment and Accessible Technology (PEAT) - PEAT is a multi-faceted initiative promoting the employment, retention, and career advancement of people with disabilities through the development, adoption, and promotion of accessible technology.
