What Services are Provided by SCHS Case Management?

Special Child Health Services (SCHS) Case Management Unit (CMU) Case Managers, with parent consent, work with the child’s parents, physician, and/or specialists to evaluate an affected child’s strengths and needs; and collaborates with the family and community-based partners to develop and individual service plan (ISP) for the child and family.

Who is eligible for SCHS Case Management?

All New Jersey residents birth through 21 years with Special Health Care Needs or at risk for Developmental Delay.

Are there income requirements?

There are no income requirements for Case Management through the SCHS CMUs.

Is there a cost for SCHS Case Management?

There is no cost for Case Management through the SCHS CMU.
