Institutional Diversion Assistance Fund (IDAF)
DDS’ Institutional Diversion Assistance Fund (IDAF) provides assistance to delay or eliminate the need for institutional care by purchasing one-time services, items or modifications with the overarching goals of:
- enabling an individual to safely remain living in the community, or
- promoting a safe discharge from an institutional living arrangement into a community setting.
The IDAF is funded through a combination of state and federal dollars.
ELIGIBILITY
Applicants must meet the following eligibility criteria:
- have a permanent, chronic condition resulting in physical disability (medical certification required upon application)
- reside in an unlicensed community setting or reside in an institutional setting with confirmed plans to move to an unlicensed community setting
- not have access to another source of readily available funding for the services, items, or modifications requested.
Note:
- the IDAF may not be used to provide on-going services
- all readily available sources of funding must be explored prior to requesting the use of IDAF
- DDS is the payer of last resort
DDS will approve one request per applicant, per fiscal year.
REFFERRAL/APPLICATION PROCESS
Referrals should be made via email to: dianna.maurone@dhs.nj.gov and contain the following information:
- applicant’s name,
- contact information for applicant or designated representative,
- brief summary of situation/need,
- other sources of funding pursued, if any.
Upon receipt, a DDS Information & Referral Specialist will contact the applicant to assist with the request.
Requests will be processed until such time that funding has been exhausted. All requests are contingent upon the availability of funding.
