DRIVER HISTORY ABSTRACT

Your driving history is available in a document called a Driver History Abstract. It includes all of your New Jersey moving violations, points, accidents involved, suspensions, etc. for the past five years.

You can request your abstract online. Please review the following information before starting.
  • A User ID #.
  • New Jersey driver license/ID.
  • Credit or Debit Card for $15 fee.

Request Your Abstract By Mail

What you need to have before starting

  • Completed Application for Driver History Abstract (Form DO-21).
  • Photocopy of your New Jersey driver license/ID.
  • Check or Money Order for $15 fee (payable to NJMVC).

Send the completed Application, copy of your driver license, and check or money order (payable to NJMVC) to:

Get Your Abstract At An Agency

What to bring to the agency

  • New Jersey driver license/ID.
  • A $15 fee to be paid by credit/debit card, cash, check, or money order (payable to NJMVC).
  • You may bring a completed Application for Driver History Abstract or fill it out at the agency.

Complete the Application for Driver History Abstract (Form DO-21) and submit it with the $15 fee at one of the counters.

The following supporting documents are available from the New Jersey Motor Vehicle Commission:

  • Order of Suspension
  • Schedule of Suspension
  • Restoration Notice, Mailing List
  • Summons
  • Accident Report

    • Complete Application for Driver History Abstract (Form DO-21). In Section B, select the document type(s) and date for each, with the reason for your request.

    Mail the Application with a separate check or money order (payable to NJMVC) for each document and a photocopy of your New Jersey driver license/ID to:
