The NJ FamilyCare Aged, Blind, Disabled Programs
The NJ FamilyCare Aged, Blind, Disabled (ABD) Programs are multiple programs for people who need help in the community or with long-term services and supports. Click here to read an Overview of the Aged, Blind, Disabled Programs.
In addition, to the programs described in the Overview, the ABD Program includes:
Medically Needy Program
This special program provides limited health coverage to aged, blind or disabled people who do not qualify for regular New Jersey Medicaid because their income or financial resources are too high. It includes a "spend down" provision that allows documented medical expense to be used to reduce monthly income in order to meet eligibility limits.
People who qualify for this Medically Needy section of New Jersey Medicaid are entitled to most Medicaid services. Exceptions include such things as inpatient hospital care, prescriptions, podiatry and chiropractic services. Click here to see what you may need in order to apply for the Medically Needy Program.
The NJ WorkAbility Program offers full New Jersey Medicaid health coverage to people with disabilities who are working, and to those whose earnings would otherwise make them ineligible for Medicaid. For NJ Workability income eligibility guidelines and more information, click here.
Medicare Savings Program
In some cases, low-income Medicare recipients may also qualify for New Jersey Medicaid, which will provide them with assistance in paying their monthly Medicare premiums, co-pays and deductibles. For information about the income eligibility requirements of this program, call 1-800-792-9745.
In determining whether a person is financially eligible, the applicant’s financial eligibility will be considered along with his or her spouse or parents. Click here to see what you may need in order to apply.
How to Apply
All of the NJ FamilyCare Aged, Blind, Disabled Programs have just one application called the NJ FamilyCare Aged, Blind, Disabled Program Application. Just complete the application, and the eligibility processor will evaluate it for eligibility. Read the information about the ABD Programs. Fill-in the application below, then print it out and mail it in. An in-person interview at the County Welfare Agency is not required to apply. If you prefer to have help completing this application, call your local County Welfare Agency. They can help you.

The ABD application booklet includes the Aged, Blind, Disabled Program Application and the supplemental Designated Authorized Representative and Spouse Information forms that must be submitted with your application, if applicable.

- NJ FamilyCare Aged, Blind, Disabled Application
- Designated Authorized Representative Form This is a supplemental form if you want to designate an Authorized Representative that will be responsible for the application and the eligibility process.
- Spousal Information Form This supplemental form is completed if a married couple is seeking eligibility through the Aged, Blind, Disabled Programs. It must accompany the Aged, Blind, Disabled Application above.
- Area Agency on Aging (AAA) Form This is an optional form. The AAA offers other services and supports that could benefit the senior applicant with disabilities. This form, when completed, gives permission to refer the applicant’s information to the local AAA for their follow up. (This form prints best on legal paper).

NJ FamilyCare Aged, Blind, Disabled Program benefits received after the age of 55 may be reimbursable to the State of New Jersey from the member’s estate. The recovery may include premium payments made on behalf of the beneficiary. For more information about Estate Recovery, click here.
