Pursuant to N.J.S.A. 17:27A-2d and N.J.A.C. 11:1-35.6, a hearing (the “Hearing”) will be held to consider the Form A Statement (the “Form A”) regarding the proposed acquisition of control of CareCentrix of New Jersey, Inc. a New Jersey licensed organized delivery system, by Blazing Star CCX Parent, Inc., Blazing Star CCX Parent Holdings, Inc., Blazing Star CCX Superco, Inc., Sycamore Partners III, L.P., Sycamore Partners III-A, L.P., Sycamore Partners Wing Co-Invest, L.P., Sycamore Partners III GP, L.P., Sycamore Partners III GP, Ltd., Stefan Kaluzny, Stefano Pessina, and Alliance Sante Participations Ltd. (collectively, the “Applicants”).   The Form A was filed by the Applicants with the Commissioner of the New Jersey Department of Banking and Insurance pursuant to N.J.S.A. 17:27A-2a.  In accordance with N.J.A.C. 11:1-35.6(g), the Hearing shall be conducted on the documents filed.  Pursuant to N.J.A.C. 11:1-35.6(f)(1), the scheduled date of the Hearing shall be seven (7) days after the publication of this notice.  Therefore, the date of the Hearing is July 30, 2025.
