How to Assign or Become a Designated Authorized Representative (DAR) For Online Access to Health Information

Medicaid/NJ FamilyCare (NJFC) deployed the MyNJFCHealthData solution to empower members by giving them access to their health information so they can make the best-informed decisions about their care, all while keeping that information safe and secure. MyNJFCHealthData can be accessed using a compatible third-party application (App) chosen by the member. To learn more about MyNJFCHealthData please read our Member Resources.

MyNJFCHealthData allows members who are 18 years or older and who are enrolled in the NJFC Fee-for-Service program but not enrolled in one the NJFC managed care organizations (MCOs) to view their NJFC claims information. Available claims information may include hospitalizations, physician visits, dental visits, vision services, pharmacy products, and other types of medical services and equipment accessed on January 1, 2016 and later.

When using MyNJFCHealthData, there may be situations where another adult individual, other than the NJFC member, may be allowed to access electronic protected health information (ePHI). This is possible by submitting a form for State approval for a Designated Authorized Representative (DAR). Assigning a DAR does not remove the adult NJFC member’s ability to access their health information using MyNJFCHealthData.

What is a Designated Authorized Representative (DAR)?
A DAR is someone who you assign to have access to your health information, like a family member or other trusted person. Some DARs may have legal authority to act on your behalf if you are determined to be incapacitated. Click here for more information.

Designated Authorized Representative for Adults (Diminished Capacity)
If the statements below are all true:
- You are requesting to become a DAR for a NJFC member who is not enrolled in an NJFC MCO.
- You are an adult who is not incapacitated
- You have legal authority to act for another adult who has been determined to be incapacitated and have the necessary documents such as a Power of Attorney or Letters of Guardianship
- You are seeking to obtain the NJFC member’s health information using the MyNJFCHealthData solution

Designated Authorized Representative for Adults (No Diminished Capacity)
If the statements below are all true:
- You are an NJFC member but not enrolled in an NJFC MCO
- You are an adult who is not incapacitated
- You are giving another adult permission to view your health information using MyNJFCHealthData

PLEASE NOTE: Approved DAR access is effective for up to one year from the date of approval unless the authorization specifies a shorter period. An approved DAR can also be revoked by filling out this form: DAR Revocation Form.
