NJ Ineligible Provider List

The New Jersey Office of the State Comptroller’s Medicaid Fraud Division works to protect the integrity of the State’s Medicaid program.

The NJ Ineligible Provider List serves as notification that an individual or entity is excluded from the Medicaid program. The list is intended to prevent Medicaid payments for care, services, or supplies, furnished, ordered, or prescribed by the ineligible provider. Use of the list for other purposes is beyond the scope of its intended use.
