Statewide Transition Plan
The Centers for Medicare & Medicaid Services (CMS) issued a final rule on January 16, 2014 to ensure that Medicaid's home and community-based services (HCBS) programs provide full access to the benefits of community living and offer services in the most integrated settings. New Jersey's Statewide Transition Plan (STP), which was submitted for federal approval in April 2015, provided a framework to bring its 1115 Comprehensive Medicaid Waiver and 1915 (c) Community Care Waiver into compliance by March 17, 2019.
CMS sent a letter requesting supplemental information on New Jersey’s proposed STP. The state's response to CMS is posted below for public review and comment period, from July 1 – August 31, 2016.
