MH/SUD Universal Incident Reporting
Please be reminded that all licensed and contracted MH and SUD Provider Agencies (PA) are required to report unusual incidents, including  operational incidents, operational breakdowns, public safety and medical (communicable illnesses) incidents into the Unusual Incident Reporting Management System (UIRMS). See e.g.  Administrative Order 2:05   see also Department of Human Services, Office of Program Integrity and Accountability (OPIA), Coronavirus Disease 2019 (COVID-19),  Incident Reporting Policies, updated November 17, 2020.

Please be further reminded that MH and SUD Provider Agencies that contract with the Division are required to report significant events that impact on the PA's organization and services to the Division's Bureau of Contracts Administration. See e.g. Policy Circular P1.11

In addition, contracted PAs should notify the Division of operational changes by using the MH form or the SUD form.   Please contact Debra Rabatie, Quality Assurance Specialist, regarding incident reporting at  Debra.Rabatie@dhs.nj.gov or Priya Nambiar, Contract Bureau Chief, regarding Fiscal Unit reporting at Priya.Nambiar@dhs.nj.gov .

Helpline for Individuals who are deaf and/or hard of hearing 


The NJ Department of Human Services’ Division of Mental Health and Addiction Services, in collaboration with the Division of the Deaf and Hard of Hearing, is pleased to announce a videophone helpline for individuals who are deaf and/or hard of hearing to access emotional support during the COVID-19 pandemic.   

Sign language users are able to communicate directly with our partners at ACCESS, of St. Joseph's Healthcare System in Paterson. Individuals who are deaf and/or hard of hearing can call 973-870-0677 VP, Monday to Friday from 9:00 am to 5:00 pm.


 
 

Consumer and Family Resources

ReachNJ is a 24-hour-a-day, 7 day-a-week addictions hotline where people who have Substance Use Disorder (SUD) or friends and family of people with SUD can get immediate assistance and support from live, New Jersey-based, trained addiction counselors.

An individual who calls the program’s toll-free number, 844-732-2465, will reach a live staff member who will help connect you to treatment.

ReachNJ will assist callers regardless of their insurance status.
