Objective 1: To increase access to and awareness of, state agency transition services and supports for eligible youth from the ages of 14 to 21, with disabilities while increasing coordination and collaboration between state agencies by creating an inclusive, user-friendly transition website. 

Objective 2: To create and provide targeted and universal inter-agency professional development to increase pathways to competitive integrated employment opportunities. 

Objective 3: To increase the effectiveness of transition services by improving post-school outcomes as outlined by increasing enrollment in post-secondary education programs and opportunities for competitive integrated employment. 

Objective 4: To create and memorialize comprehensive transition networks to establish an effective systematic change in partnerships and collaboration; and, 

Objective 5: To seamlessly coordinate efforts across state agencies, ensuring that all students with disabilities have access to meaningful transition supports.
