Please choose the "Fillable Transcript Order Form" or "Fillable Audio Recording Form" on the right under Documents and follow the directions to complete.

PLEASE NOTE: The either request form must be submitted electronically to the OAL via email to oaltranscripts@oal.nj.gov.  It is preferred that requestors download the form to their computer and complete it electronically and submit via email or fax (requestors can print the form and scan and email or fax it to OAL).  If you are requesting a transcript, you still must submit the printed form with the appropriate deposit to your chosen vendor.

SPECIAL EDUCATION PROCEEDINGS

If you are requesting a transcript for a special education proceeding, you may obtain one for free.  Kindly make your request directly to the Department of Education, Office of Special Education at osepdisputeresolution@doe.nj.gov or Baljit Kaur at baljit.kaur@doe.nj.gov. Be sure to include the following information.  No form is required:

•   The case name
• The OAL docket number (which begins with EDS)
• The agency reference number
• The dates for which you want a transcript
• The name of the Administrative Law Judge
• The format of the hearing (virtual or in person) for which you want a transcript
