Increasing Transparency of One-Star Nursing Homes

15 long-term care facilities (LTCs), commonly known as nursing homes, that receive Medicaid dollars have been repeatedly rated one-star for the quality of care residents receive. The Office of the State Comptroller (OSC) oversees the integrity of New Jersey's Medicaid program, and set out to examine the care that some 1,850 New Jerseyans receive at one-star LTCs.

These ratings are scored by the Centers for Medicare & Medicaid Services (CMS) within the federal government's Department of Health and Human Services. Ratings are based on health inspections, quality measures, and staffing. 

Our list below selected LTCs that were rated one star out of five for at least six of the last eight quarters. Read more about our selection methodology.
