PROJECT CONNECT helps to assure your family is connected to services in the community after your youth is discharged from a hospital following a suicide attempt or suicide ideation. A Regional Care Coordinator will reach out to your family to provide support and access to services. 

For assistance with behavioral health and to access Mobile Response Stabilization Services for crisis situations, call PerformCare (1-877-652-7624). PerformCare is available 24/7 to connect youth and families to individualized care for behavioral health, intellectual/developmental disability, or substance use treatment needs.
