Respondent, Lisa Cannon, knowingly wrote and filled fictitious prescriptions using her husband Stuart Ramson's name and insurance.  Horizon Blue Cross Blue Shield paid a total of $640.12 for pharmacy claims submitted between April 1, 2008 and May 8, 2009. Claims were also submitted to Horizon Blue Cross Blue Shield for Harold Mager for date of service July 13, 2011 and Raymond Gruber for date of service August 8, 2011 after Lisa Cannon's medical license to practice in the State of New Jersey was expired on June 30, 2011. Lisa Cannon obtained $332.41 in claim benefits for which she was not entitled.

On June 6, 2012, this vehicle was involved in an auto accident and it was discovered that the vehicle was being used as a taxi cab in a commercial enterprise. Therefore, the insured made a material misrepresentation on an insurance application by failing to disclose that the 2008 Toyota Prius was being used commercially.  If IFA Insurance Company was aware of the usage of vehicle they would not have insured it as they do not insure livery vehicles.
