Lost or Stolen Licenses/How To Get A Duplicate

You can only get a duplicate license or non-driver ID as a replacement if yours was lost, stolen or destroyed. You cannot drive until you receive the replacement license. Using a duplicate driver’s license as a second or spare copy is strictly prohibited. No driver may hold multiple copies of a license or a permit.

Visit any motor vehicle agency with 6 Points of ID and proof of address.

  • Complete a form BA-208 for Driver’s License or for BA-207 for Non-Driver ID (found only at motor vehicle agencies).
  • Pay the $11 fee.

When obtaining a duplicate for a lost or stolen driver license, you may be required to take a new photo. We are required to capture a customer’s full face when taking a picture for a driver license or ID Card; therefore we need to be able to see from the top of your forehead to the bottom of your chin. If you have a medical or religious need to wear a head covering in your photo, please advise the MVC staff at the agency camera station.

Note:  Stolen licenses or IDs should be reported to the police. If you are out of the state for an extended period, and unable to appear in person follow these special instructions.
