All bidders must submit documentation to centralprocurement@doh.nj.gov. Please include the RFP/RFQ number in the subject line of all emails when referring to a specific procurement. Failure to do so could result in missed emails due to volume.
Bidders are advised that each procurement may have different submission requirements. The Vendor Form/Certification Checklist provided on this page is a guide to assist bidders in completing their quote and is general in nature. Bidders must refer to the RFP/RFQ to review all submission requirements specific to that procurement. The most common forms and certifications are provided on this page for convenience.
