Long-Term Care License Application Fees
Important Note: Applications without proof of payment will not be processed.
License Types:
1. License Initial (LI):
- Determine the fee by selecting Facility Type and Application Type below.
- Email required documents and proof of payment to CNLapps@doh.nj.gov.
- Additional documents may be needed during Functional and Application Review.
2. License Amendment (LA):
- Determine the fee by viewing Facility Type and Application Type below.
- Email required documents and proof of payment to CNLapps@doh.nj.gov.
- Additional documents may be requested during Functional and Application Review.
3. License Renewal (LR):
- Renew online at dohlicensing.nj.gov/SignIn.
- Create an account, renew your license, and pay the fee simultaneously.
- The online portal is safe and convenient for managing multiple facilities.
- To determine renewal fee, go to the portal.
Renewal Process:
- Access the portal at dohlicensing.nj.gov/SignIn.
- Register at dohlicensing.nj.gov/registrationrequest/.
- Complete the renewal application and data sheet online.
- Corrections to the facility data sheet are limited to specific fields.
Required Documents (for LI and LA Applications):
- Application form: Completed, signed, and dated. Find the form for your facility type here.
- Facility Details: Legal name, DBA/Facility name, address, official email, and phone number.
- Management Company (if applicable): Company name, address, phone, and email.
- Application Project Narrative: On official letterhead, describe beds, services, and facility details.
- Proof of payment must accompany all applications.
- Application Contact Information: Name, phone number, and email.
Reminder: Send required documents and proof of payment to CNLapps@doh.nj.gov.
Note: License fees are non-refundable.
