The Jersey City Office of the City Clerk - Division of Vital Records is now authorized to issue birth certificates for anyone born in Jersey City after 1925. These certificates will be accepted by the N.J. Motor Vehicle Commission for Real ID applications and by the U.S. Department of State for passport applications. 

Requests for birth certificates can made in person, by appointment only, or through the mail. To make an appointment to apply for a birth certificate in person, call 201-547-5150.
