  PLEASE NOTE: The either request form must be submitted electronically to the OAL via email to oaltranscripts@oal.nj.gov.  It is preferred that reequestors download the form to their computer and complete it electronically and submit via email or fax (requestors can print the form and scan and email or fax it to OAL).  If you are requesting a transcript, you still must submit the printed form with the appropriate deposit to your chosen vendor.

If you are requesting a transcript of a Special Education matter (docket number starts with EDS), you MUST contact the Department of Education to process you request.
