[SRRA]: Potable Well Information Form Template Now Available on the Site Remediation Program Web Page

A Potable Well Information Form template for LSRPs involved in conducting potable well testing at private properties is now available on the Site Remediation Program's web page at https://www.nj.gov/dep/srp/guidance/IEC/.

The Potable Well Information Form template was developed to be used in conjunction with the potable well access letters that are already available on the Site Remediation Program web page. Like the potable well access letters and potable well sampling result letters templates, the Potable Well Information Form template is provided as a courtesy and its use is optional.
