Environmental and Occupational Health Assessment Program Application Payments

Paying for a license, registration, certificate, permit or penalty via e-check or credit card:

Directions: Click on the type of certification you wish to pay for from the selections below. Once you have submitted payment, you must complete and submit the required form and (if applicable) supporting documentation before our review process can begin.

Trade or Individual Name: Enter the name of the lead certification applicant, training agency or child care center that the payment is for.

Pertinent Number: This is your license/permit number.

Important: Online payment alone is not sufficient for our review process to begin. The original, signed forms and any required attachments and/or documentation must be mailed to this office with the proof of online payment attached. Your submission will not be valid until the required forms and documentation have been received by this office.

When you are finished paying:

Print the payment confirmation page or the confirmation e-mail you received as proof of payment. Attach this printout to your application form. No submission will be processed without proof of payment.

Please note that the application fees listed below are non-refundable.

Lead Permit Application Payments

Both initial and renewal applications fees are as follows:
$80 – Workers for Housing and Public Buildings and Workers for Commercial Buildings and Superstructures.
$150 – Supervisors for Housing and Public Buildings and Supervisors for Commercial Buildings and Superstructures.
$150 – Inspector/Risk Assessors
$200 – Lead Planner/Project Designers
$25 – Lost Lead Permit

Asbestos & Lead Training Provider Certifications

These fees are for companies who want to conduct asbestos and/or lead training courses in NJ. The fee is $500 per discipline (initial and refresher course applications are considered separate disciplines).

Indoor Environmental Consultant Applications

The fee for both initial and renewal consultant applications is $2,000.

Certified Indoor Environmental Consultant Applications

Child Care Centers Applications

The fee is $1,500 for Initial, New Owner, Centers with Environmental Hazards (which require regular monitoring and testing), and some Expansion applications.

The basic fee for renewal applications is $450, which can be paid when submitting the renewal application. However, if environmental hazards are identified, centers will be required to pay the full $1500 fee. The inspector will inform you of any additional fees once a review is performed of your renewal application.
