Diabetes
To improve prevention and control of diabetes in New Jersey, The New Jersey Diabetes Prevention and Control Program facilitates grant funding for Federally Qualified Health Centers (FQHCs) and other eligible health care organizations that successfully apply to become Diabetes Resource Coodination Centers (DRCCs).
About DRCCs
DRCCs promote diabetes self-management education (DSME) for New Jerseyans diagnosed with diabetes, as well as lifestyle intervention programs for residents at risk for type 2 diabetes. Local health departments, hospitals, FQHCs, universities, and non-profit organizations with 501(c) 3 status may apply to become DRCCs.
DRCCs collaborate with healthcare providers to increase patient participation in diabetes self-management and prevention programs. In addition, DRCCs implement culturally and regionally-specific media and communications systems to publicize the availability of these programs. Successful DRCCs have proven capacity in provider engagement, program promotion, and health communications.
