REQUIRED DOCUMENTS

IMPORTANT: The deadline to submit missing documents to complete your application has passed.

If you submitted documents, we are reviewing your application and will be in touch with you as needed. Please also monitor your text and emails for updates regarding your application.

Applicants can demonstrate that they were excluded from both the federal stimulus checks and the COVID related unemployment assistance with any of the following ways:

  • Bank statements for the months of April & May of 2020, December 2020 & January 2021, and April 2021 & May 2021, or
  • Complete 2020 Tax Returns demonstrating that applicant(s) did not receive any federal stimulus. Receipt of the federal stimulus only applies to the applicant(s); therefore, if another member of your household for example, your child received this payment, it will not preclude you from applying as long as you did not receive the benefit.

Applicants who do not have any of these documents may still be eligible. All applicants are required to submit a signed attestation of exclusion from federal and unemployment pandemic assistance.

This program is paid with federal funds and it is unlawful to provide any false or misleading information to receive these benefits.

Applicants will also have to demonstrate that their household gross income is at or below $55,000 by providing ONE(1) of the items listed below.  Gross income is the total of all wages, salaries, profits, interest payments, rents, and other forms of earnings before any deductions or taxes.

  • Joint or individual, most recent (2020/2021) tax return or W2s showing household income at or below $55,000, or
  • Paystubs (December 2021 paystub showing year-to-date or paystubs for thelast two months if no year-to-date), or
  • Letter from Employer (Optional template available), or
  • Award Letter from a Social Service Agency (example - Social Security or Veteran Affairs), or
  • Deposit record or bank transaction sheets with total balance for the last 2 most recent months, showing annual income of less than $55,000), or
  • Letter from a nonprofit, 501c3 organization that has provided services to the applicant (Instructions here).

Applicants who do not have any of these documents may still be eligible. An application reviewer can assist you identify other ways to demonstrate your annual household income.

Applicants must present one document from List A or a combination of one (1) selection from list B and one (1) selection from List C to prove Identity and Residency. All applicants must be 18 yrs. of age and reside in the State of New Jersey.

  • New Jersey or Municipal ID card with full name and address

  • New Jersey Driver’s License with full name and address

  • Student identification card from a school located in New Jersey

  • Utility bill, cell phone bill, or internet bill showing your name and address

  • Bank statement or insurance statement showing your name and New Jersey address

  • 2019/2020 Tax Return showing name and New Jersey address

  • Paystub from employer showing your name and New Jersey address

  • Rent receipt, lease, mortgage statement, or property tax statement showing your name and address

  • Letter from social service agency, health care provider, or government agency showing name and New Jersey address from the past 12 months

 

 


  • Student identification card from any school

  • Passport from any country

  • Consulate identification card

  • Birth certificate from any country

  • A child’s U.S. birth certificates listing the applicant as a parent

  • Work issued identification

  • International driver’s license

  • Individual Taxpayer Identification Number (ITIN) issuance letter from the IRS or ITIN card

  • Department of Corrections ID

  • Marriage certificate or divorce record from any country

  • High school or college transcript from any country

  • U.S. court documents where the applicant is named as a party

  • U.S. Union ID

  • U.S professional license

 

Signed and dated letter stating the full name and phone number of the person writing the letter from the following entities (Optional template available):

  • Landlord acknowledging New Jersey residency

  • Representative from your place of worship

  • Medical provider, service provider, or shelter
