Purpose: It is the intent of the Preparing Students with Individualized Education Programs (IEPs) for Life in the Community, Integrated Employment and Post-School Success (Year 2 of 3) to enable LEAs to continue to develop model programs for students with disabilities aged 16-21 that implements transition-focused services within the local community based on the student's IEP. Program service options would include: person-centered planning, community-based instruction; and systematic instruction during age-appropriate activities in the community, and facilitating linkages between students and adult service agencies. Funds are intended to supplement the LEA�s efforts to provide Free Appropriate Public Education (FAPE). The New Jersey Department of Education (NJDOE) has established a three-year grant program which focuses on the development of model programs. This grant program is being funded using federal IDEA funds. This grant program is open to LEAs or consortia of LEAs that were selected through a competitive process in Year 1. Based on the availability of federal resources, this second year of the three year grant program will begin September 1, 2017 and will end on August 31, 2018
