  • You will need proof of New Jersey residency, this can be a driver's license, a state ID or a copy of a utility bill or government correspondence
  • You will need the three (3) most recent savings and checking bank statements and any other accounts you may have
  • Documentation if you have a person designated as your Power of Attorney or Guardian

Yes, you must provide your spouse’s bank statements and statements from any other accounts they may have.

  • This is an online process, and your doctor will need to fill out the medical documentation. Please call your doctor’s office and request their email address or the office manager's email address
  • When calling the doctor’s office, please tell them the medical documentation requirement is not done over email. The email is only used to send the link to the secured portal where the forms are located.  Nothing containing protected information is done over email
  • If you are having difficulty getting an email for your healthcare provider, please contact the TBI Fund at DDS-TBI.Applications@dhs.nj.gov or 1-888-285-3036
  • Do not make up an email; this will cause your application to be canceled
  • Do not put yours, a family member's, a friend's, or a case manager’s email as the healthcare provider’s email, as this will cause your application to be canceled

Unless you have a Power of Attorney or a Guardian, you, the applicant, must sign the application.

  • If you make up an email for your healthcare provider
  • If you do not upload the most recent 3 months of bank statements
  • If you upload the same bank statement 3 times

Unfortunately, at this time, there is no way to save the information and submit it at a later date.

Yes, even if the lawsuit has not been settled, you will need to provide your attorney’s contact information in the application. If a lawsuit is filed or becomes pending at any time, even after you submit your application, you must notify DDS by calling 1-888-285-3036 and selecting prompt # 1 or by emailing Dhsco.DDS-TBIFund@dhs.nj.gov.

No, there is no age limit on who can apply to the Fund.

A traumatic brain injury is defined as an injury to the brain caused by a blow or jolt to the head or a penetrating head injury/neuro-trauma that disrupts the normal brain function, where continued impairment can be demonstrated.  This definition does not include dysfunction caused by congenital or degenerative disorders, birth trauma, acquired brain injuries (stroke, aneurysm, etc.) or injuries caused by other circumstances.
