Priority #2: Promote Connectedness and Improve Care Transitions
Overview
Promoting connectedness is one of the most effective upstream strategies in Veteran suicide prevention. When Veterans feel supported by family, peers, and their broader community, their sense of belonging increases and suicide risk decreases. Because many Veterans at risk are not engaged in VA care, building partnerships with community providers, peer networks, and local organizations becomes essential. These relationships help identify Veterans earlier and offer support in the environments where they live, work, and thrive - before they reach a crisis point. Connectedness also strengthens the handoff between systems, making it easier for Veterans to stay engaged with care and resources.
Strengthening care transitions is equally critical, as these moments often carry the highest suicide risk. Leaving military service, stepping down from inpatient or emergency care, relocating, or navigating homelessness or justice involvement can disrupt routine and disconnect Veterans from needed support. Evidence shows that warm handoffs, timely follow-up, and caring contacts significantly reduce suicide attempts and deaths. By coordinating these transitions across systems and aligning them with community connectedness efforts, we help prevent Veterans from falling through gaps and ensure they maintain access to consistent, lifesaving resources.

Goals
- Strengthen transitions for Service Members by partnering with leadership to provide outreach, resource navigation, and early identification during the shift out of military service.
- Expand non-VA community engagement through collaboration with hospitals, behavioral health agencies, peer support programs, and faith-based organizations to identify Veterans, coordinate warm handoffs, and reduce service silos.
- Build and support peer-led connectedness initiatives, including Veteran-to-Veteran groups, mentoring networks, and community-based check-ins that increase belonging and provide regular touchpoints outside clinical settings.

- Engagement with Service Members with the Medical Engagement Board to improve transitions for those leaving military service.
- Improve care transitions for Veterans in the community through sustained partnership with VA and non-VA medical centers and community healthcare systems.
- Promote connectedness by supporting peer programs and coordinating available services across organizations to reduce silos and strengthen resource navigation.

VSO's / Outreach Schedule
Through our network of regional Veterans Service Offices (VSO), the NJ Department of Military and Veterans Affairs (DMAVA) provides the state's Veterans and their dependents with information and guidance in filing claims. Trained Veterans Service Officers at these offices also assist Veterans with issues pertaining to employment, education, burial, counseling, housing, social and medical services, and other areas of concern to Veterans and their families. DMAVA also conducts Outreach Events in the community to help and assist any Veteran.

The New Jersey National Guard Family Programs Office in Bordentown operates eight Family Assistance Centers around the state to serve the needs of Servicemembers and their families. Whether pre-, during or post-deployment, these centers offer information and referral services to all Service Members and their families regardless of their branch of service or duty status.

Do Not Walk Alone
ETS - Expiration of Term of Service - should be an exciting opportunity for the Service Member who is opening the door to the next chapter of life. Yet, often, the stresses associated with walking through this door are challenging. The ETS Sponsorship Program ensures that the Service Member - and the family - do not walk alone. Our sponsors, who live in the community, will not only walk through the door with the Service Member, but will continue to coach and guide throughout the transition process.

New Jersey families suffering from behavioral health crises, chronic mental illness and substance use disorders can benefit from peer family support and education. The NJ Self-Help Groups Clearinghouse can connect you with opportunities for family members to join self-help groups, receive education and training about these disorders as well as gain the support and experience from those with the same life experience.

Warriors Welcome is a no-cost, judgement-free, veterans-facilitated zone. Virtual meetings are held the 1st and 3rd Fridays at 6:00pm. This program is sponsored by the Mental Health Association in New Jersey.
