Reporting Requirements in New Jersey
New Jersey STD Reporting Requirements (PDF)
Persons required to report
- Physicians
- Advanced practice nurses
- Physicians Assistants
- Person having control or supervision over a hospital/health care facility, correctional facility, school, summer camp, childcare center, preschool, or institution of higher education.
- Laboratories (for specific guidelines for labs, please see NJAC 8:57-1.7 )
Reportable STDs
- Chancroid (Haemophilus ducreyi)
- Chlamydial Infections, sexually transmitted (Chlamydia trachomatis)
- Chlamydial Infections, neonatal (Chlamydia trachomatis)
- Gonorrhea (Neisseria gonorrhoeae)
- Granuloma inguinale (Calymmatobacterium granulomatis)
- Lymphogranuloma venereum (Chlamydia trachomatis)
- Syphilis, primary and secondary (Treponema pallidum)
- Syphilis, congenital
Required Information
- Name of disease
- Personal information for the person infected:
- Name
- Age and date of birth
- Address and telephone number
- Gender
- Race
- Ethnicity
- Symptoms
- Treatment (medication, dosage, and dates)
- Name, address, direct phone number, and institution of reporting official
How to report an STD to NJDOH
- STD reports should be made directly into the Communicable Disease and Surveillance System (CDRSS)
- Email cdrs.admin@doh.nj.gov if you have any questions.
- If unable to report through CDRSS, reports can be made:
- By phone to the STD Program 609-826-4869
- Via secure fax using one of the STD-11 Confidential STD Report Forms below.
Forms
Expedited Partner Therapy (EPT)
N.J.S.A. 26:4-48.2 (the EPT statute), authorizes a health care professional to give patients diagnosed with an STD medication or prescriptions to give to their sex partners without conducting a medical evaluation of the patient’s sex partners.
