Annual Accident Reports

After the last day of camp, each registered Youth Camp is required to submit an Annual Accident Report (AAR) to the Youth Camp Safety Project whether or not incidents are reported. The Annual Accident Report lists illnesses and injuries sustained by campers during the camp operating season.

 

Forms and Submission

After the last day of camp, complete each step below.

1. Save a separate Annual Accident Report (AAR) form CB-15 for each registered Youth Camp

The Annual Accident Report form CB-15 is a fillable PDF. Please download the form and save it to your computer.

Complete one separate AAR for each Permanent Camp ID Number.

2. Input all major injuries and illnesses

The AAR form for reporting accidents resulting in serious injury or death must be submitted annually by each youth camp to the Department. Serious injuries include head or neck injuries, loss of consciousness, fractures, early termination of a camper's stay, the need for professional medical treatment or transport to a medical facility, or death. Minor injuries, requiring only first aid and not involving any of these conditions, are not subject to this reporting requirement.

If no reportable accidents occurred, you are still required to submit the form. Select the checkbox on the form that indicates no incidents to report.

 

3. Submit the form via email

Save the completed form to your computer and attach it to a new email.

In the subject line of the email, include Annual Accident Report for Camp ID # [your camp ID]
