Providers are required to verify that any current or prospective employees (regular or temporary), contractors or subcontractors, who directly or indirectly will be furnishing, ordering, directing, managing or prescribing items or services in whole or in part are not excluded, unlicensed or uncertified by searching the following databases on a monthly basis:

Mandatory

• Federal Exclusions Database
• State of New Jersey Debarment (ineligible provider) Database
• NJ Treasury's Debarment Database

Mandatory, if Applicable

• NJ Division of Consumer Affairs Licensure Verification System (includes all licensed healthcare professionals)
• NJ Department of Health Licensure and Certification Database (includes Nursing Home Administrators, Certified Assisted Living Administrators, Certified Nurse Aides/Personal Care Assistants, and Certified Medication Aides)

Optional and Fee-Based

• National Practitioner Data Bank (only certain provider types may access this database - register here)
