The NJ FamilyCare Housing Supports Provider Readiness Program (HSPRP) for State Fiscal Year 2025

The New Jersey Department of Community Affairs (DCA) and Division of Medical Assistance and Health Services (DMAHS) at the Department of Human Services (DHS) are pleased to announce a new funding opportunity: The NJ FamilyCare Housing Supports Provider Readiness Program (HSPRP) for State Fiscal Year 2025.
Available Funding: $10,000,000 total
Maximum Award: $250,000 per organization
Grant Period: January 15, 2025 - January 14, 2026
Application Deadline: December 16, 2024, by 11:59 PM
Program Overview: This program will provide essential funding to organizations preparing to participate in the NJ FamilyCare Housing Supports program.
Funding can be used for:
- Startup costs (Medicaid enrollment, MCO contracting, IT systems)
- Seed capital for new staff (administrative and frontline)
- Program readiness activities
- Flexible funding for innovative approaches
Important Requirements:
- Minimum three years of experience in providing relevant housing services
- Attendance at mandatory technical assistance workshop
- Active participation or willingness to participate in NJ HMIS
- Executed Letter(s) of Intent with MCO(s)
- Demonstrated commitment to Housing First principles
- Inclusion of persons with lived experience and expertise (PWLEE)

How to Apply: Applications must be submitted through DCA SAGE. Complete application guidelines and requirements can be found in the full RFP, attached.
Funding Distribution:
- 30% upon grant selection
- 30% after completing first milestone group
- 30% after completing second milestone group
- 10% after completing remaining milestones

We strongly encourage eligible organizations to attend the Camden Coalition/Corporation for Supportive Housing "Medicaid 101" webinar on 11/7 from 11:30 to 1pm, prior to attending the mandatory technical assistance session to learn more about this opportunity and the application process.
