The Division of Mental Health and Addiction Services (DMHAS) has been managing the registry of Psychiatric Advance Directives (PADs). DMHAS contracted with U.S. Living Will Registry (USLWR) to manage and maintain a voluntary, secure internet based registry for PADs. You may access the registry here.
Use this secure tool to store your PAD and ensure your mental health care wishes are honored if you are unable to manage your own care.
This secure registry allows you to store your PAD so that mental health care providers and anyone else you grant access will honor your wishes.
The database of PADs at this website is held in a confidential and secure site, protected by the most up-to-date web and database security standards. All information submitted will be used only by authorized individuals and will be kept completely confidential. This information will not be shared or sold.
