Each long term care (LTC) facility has been contracted with their linked provider for the medical appointment transportation of their resident Medicaid members and there are ModivCare regional facility liaisons as listed below to address any transportation issues of the LTC facility residents in their region. All LTC facility administrators need to escalate their residents’ transportation concerns to the appropriate liaison staff in their area and if issues are unresolved escalate to the staff at SMU listed above for assistance.
