The Ineligible Provider List serves to provide notice that an entity and/or an individual is excluded from the New Jersey Medicaid program. Such exclusion can be termed a “Disqualification,” “Suspension,” “Debarment,” ”Termination,” or some combination of these actions. This List further provides notice that no NJ Medicaid funds shall be used to pay for any item or service that an excluded provider furnishes, directs, orders, prescribes, manages or supervises, or for which such entity and/or individual is involved in any capacity.
