Consultant Requirements for Prequalification
Firms seeking to be prequalified with NJDOT for the first time, must complete the PS-12 application Form. Firms currently prequalified, that have a "prequalification expiration date" indicated in the Prequalification Status Report, please submit your application PS-12 prior to this expiration date.
Starting 10/1/16 disciplines E-5, E-7, L-1, and U-2 are project specific and cannot be submitted for renewal.
Starting 10/1/16 G-2 is a primary discipline and can be submitted with renewals or at any time.

Prime Consultant Requirements
To be eligible to submit a Technical Proposal, a firm must meet the following requirements:
- Have a current prequalification on file and be approved
for at least one discipline.
- Have the firm's accounting system approved by NJDOT
(Cost Basis Approval)
- The Firm must also include a company check, cashier's
check or money order in the amount of $100.00 payable
to "Treasurer, State of New Jersey." No application
will be processed until the company check, cashier's check
or money order is received.
A firm must meet the following requirement:
- Have the firm's accounting system approved by NJDOT. (Cost Basis Approval)
Submitting the Prequalification Form
Completed forms should be submitted - (1) Hard Copy and (1) CD Copy to:

Prequalification packages must be submitted at a minimum, two (2) business days prior to any proposal submission date to allow for proper review and approval of the package.
Failure to meet this time requirement may result in rejection of a consultant's technical proposal for not being prequalified for a solicitation.
After Application Submission
Prequalification forms will be reviewed by the Bureau of Professional Services. Consultants will be notified of the results.
