Temporary Disability Insurance (TDI) Year-End Statements

Temporary Disability Insurance (TDI) year-end statements will be available online via the Department of Labor's Temporary Disability website in January for the preceding calendar year.
Before you proceed, you will need the following:
- Your Federal Employer Identification Number (FEIN), formatted to be 15 digits, with a zero before the first number, and five zeros after the last one. For example, if your FEIN is 123456789, you must enter it as 012345678900000.
- The permanent authorization code issued by the Department of Labor (not to be confused with the PIN used to file your NJ-927 and WR-30 with the Division of Revenue). If you got an authorization code by mail in the last two years, use that same code in 2020 to access your year-end statement.

If you can't find your authorization code, use the verification application in Employer Access to retrieve it.
Before you do so, you will need the following:

- Your official business name, as it appears on forms NJ-927 and WR-30 (no abbreviations).
- The amount from the prior quarter of the total of all wages paid that are subject to Unemployment, Temporary Disability, Workforce, and Family Leave Insurance (line 8 of the State's form NJ-927)
