DDD Resource Team

Sample Tracking Sheets

The DDD Resource Team developed sample tracking sheets for collecting medical and behavioral information relevant to a person’s care. These are for provider awareness. Providers may use similar data collection sheets instead of or along with these sample sheets.

The ABC Data Sheets are one example of a data collection system. All ABC data collection should be individualized and only implemented by a trained behavior support professional such as a BCBA. This data collection system should not be used as a standalone assessment.

Prevention Handouts

The DDD Resource Team and DHS Office of Public Affairs collaborated on prevention handouts that address common health and safety issues for individuals with intellectual/developmental disabilities.

Mental Health Resources

Crisis Assessment Response and Enhanced Services (CARES), administered through Trinitas Regional Medical Center Behavioral Health/RWJ Barnabas Health, provides crisis response and stabilization services for a period up to 120 days to adults (age 21 and older) with intellectual and developmental disabilities (I/DD). CARES staff are located in regional offices and respond anywhere in New Jersey to assist individuals with I/DD  who are in mental health or behavioral crisis. CARES can prevent unnecessary hospitalizations by helping manage behavioral crises in place, and assisting individuals, family members and professional staff obtain needed expertise to support someone in crisis. CARES referrals can be made by calling 1-888-393-3007, 24 hours a day, 7 days a week.
