Safety in Your Setting Month - November 2025
The DDD Resource Team and Office of Risk Management are collaborating to offer Safety in Your Setting Month.
Through a series of trainings and roundtable discussions, they will provide information for agency staff and caregivers about supporting the safety of people with intellectual and/or developmental disabilities in whatever setting they are in.

Sample Behavioral Data Collection Sheets
One example of a data collection system is the ABC Data Sheets. It is important to note that all ABC data collection should be individualized and implemented by Board Certified Behavior Analysts (BCBAs) for use by staff. This data collection system should not be used as a standalone assessment.
Functional Behavior Assessment (FBA) and Behavior Support Plan (BSP) Checklists
The Functional Behavior Assessment (FBA) is essential for developing a Behavior Support Plan (BSP). This concise guide is designed for Board Certified Behavior Analysts (BCBAs) to align assessment findings with support planning. It ensures that strategies are individualized, evidence-based, and tailored to meet each individual's needs, leading to more effective interventions and outcomes.
Behavior Support Progress Notes
Behavior Support Plan (BSP) progress notes are used to document the implementation of the plan, the individual's response to interventions, and the effectiveness of the strategies employed. These notes serve as a record to ensure staff follow the plan correctly and track progress toward behavioral goals. By noting key observations, interventions, and results, BSP progress notes enable informed decision-making, guide necessary changes to the plan, and ensure compliance with agency and regulatory standards for quality assurance.

The DDD Resource Team and DHS Office of Public Affairs collaborated on health and safety bulletins that address common issues for individuals with intellectual/developmental disabilities.

Mental Health Resources
Crisis Assessment Response and Enhanced Services (CARES), administered through Trinitas Regional Medical Center Behavioral Health/RWJ Barnabas Health, provides crisis response and stabilization services for a period up to 120 days to adults (age 21 and older) with intellectual and developmental disabilities (I/DD). CARES staff are located in regional offices and respond anywhere in New Jersey to assist individuals with I/DD who are in mental health or behavioral crisis. CARES can prevent unnecessary hospitalizations by helping manage behavioral crises in place, and assisting individuals, family members and professional staff obtain needed expertise to support someone in crisis. CARES referrals can be made by calling 1-888-393-3007, 24 hours a day, 7 days a week.
