Incident Reporting Forms and Instructions
Submit initial and follow-up incident reports (IR) to the Office of Risk Management (ORM) by uploading them through DDD UpDoc, our secure upload application. Please review the UpDoc Instructions if you are not familiar with how to use DDD Updoc.
