Making Changes to or Requesting a Replacement Exempt Organization Certificate (Form ST-5)

If you need a copy of your Form ST-5 or need to update the organization's name or physical location, follow the steps below. If you need to change the mailing address or make any other business registration changes for the organization, contact the New Jersey Division of Revenue and Enterprise Services (DORES).

Replacement or Duplicate Forms

If you need a replacement or duplicate Form St-5, complete the New Jersey Exempt Organization Request for Updated or Replacement ST-5 form . The form must be signed by an organization officer and include the tax exemption number.

Name Changes

For name changes, complete the New Jersey Exempt Organization Request for Updated or Replacement ST-5 form . In addition, you must include a copy of the amendment to the certificate of incorporation or constitution with your request.

Address Updates

If you need to update the organizations' physical location and the organization is not registered for any other New Jersey tax types, complete the New Jersey Exempt Organization Request for Updated or Replacement ST-5 form . You cannot use this form to update a mailing address. This includes P. O. Boxes.

If the organization is registered for New Jersey Employer Withholding or Sales Tax, you should use DORES On-Line Registration Change Service to update the organization's address. Once DORES updates their information we will issue an updated ST-5.
