Parental Consent

Effective December 15, 2026, the below forms are to be used by LEAs that need to get consent from a parent/guardian. In addition to English and Spanish, the form, annual notification, and FAQ document are also available in the next ten (10) most commonly spoken languages of parents/guardians in the State. The parental consent form is the same for both SEMI and GEMI students.

*Please Note: Consent provided on the discontinued SEMI parental consent form (dated 2017) remains valid for SEMI students for as long as they are enrolled in the district that obtained the signed consent form.

Inventory of Translated Vital Documents

Translation of vital documents is an ongoing effort. As of the publication date, the following vital documents are publicly available in the languages specified for each division/office. Please continue to use the SEMI Annual Notification form until the new SBHS annual notice is published. An icon or indicates availability to download.
